Certificate Request/Solicitud de Certificado

To request a copy of your Sacramental Certificate, please fill out the form as thoroughly as possible.

Please indicate if you need the certificate to be mailed to you or if you will pick up your certificate in our Pastoral Center office.

There is a $20 fee for each certificate requested, payable by cash or debit/credit card at the office.

Certificates take 4-5 business days to process. If you request your certificate by mail, please take into account additional processing time.
